Last month Apple had issued a recall for the Ultracompact USB Power Adapter that
were shipped with every iPhone 3G in the form of an exchange program.
Folks at MacMerc have disassembled one of those faulty Ultracompact USB Power Adapter to find out the reason that prompted the recall.

Apple had mentioned at the time of announcing the recall that under certain conditions the ultracompact USB power adapter’s metal prongs can break off and remain in a power outlet, creating a risk of electric shock.
If you checkout the photos of the disassembled Ultracompact USB Power Adapter below it is quite clear why Apple decided to recall the adapter.
The prongs only extend into the adapter about 1/8 of an inch and are held in place with some type of glue which MacMerc points out is more a case of "wishful thinking".
Apple had reported that they had got complaints from a small percentage of iPhone 3G users. The reason for that could be because many iPhone 3G users do not use the ultracompact USB power adapter that often as based on the details and the photos there should have been many more complaints.
It will be interesting to find out how Apple has fixed the issue in the replacement adapters to prevent the prongs from breaking off. We will keep you posted as soon as we get more details.
